Aims. Providing nutrition for a patient, when eating the usual way is impossible.

Tasks. Crating an artificial opening into the antrum of the stomach through the front abdominal wall.

Methods. The front wall of the gastric is being stretched out in the form of a cone and sutured to the abdominal wall. The gastric fistula is paved all over with mucous membrane.

Stages of work.

In 1842 V. A. Basov was the first in the world to perform gastrostomy on lab dogs.

An artificial fistula in the dog’s gastric was created. The aim of the experimental surgery was the research of the secretory and motion function of the gastric under the conditions of chronical experiment. V. A. Basov has performed 8 such surgeries.

Implementation. The surgery, depending on the kind of the case (tumour, cicatricial narrowing, ets.) may be performed with different aims: (for patient feeding, for retrograde gullet bougienage, during plastic surgeries on the esophagus, during radiation and combined therapies in cases of inoperable esophagus cancer, as a preparation stage for plastic surgeries on esophagus on weakened, malnourished patients, ets.).

Value.

The procedure of the first gastrostomy worked out by V. A. Basov in 1842 was a starting point of surgical treatment of esophagus and cardiac orifice cancer. More than a hundred modifications of this surgery has been suggested since. Nowadays gastrostomy is the most common among the palliative surgeries, used in cases of esophageal obstruction both in Russia and abroad.
